New Patient Registration
Hampton Hill Medical Centre accepts new patient registrations from patients from the surrounding areas (TW2, TW11 or TW12 postcode). Please check at reception for further details or look at our catchment area here.
Anyone who lives within our practice boundary can register online or come in to the surgery.
New patients must fill out BOTH the new patient registration form AND the medical questionnaire (after carrying out a self-assessment health check in practice). We also request photo ID and a recent (within 3 months) proof of address. All new patient registration paperwork is to be completed before we can accept you.
If you are registering a child under 6 years, please ensure that you bring their immunisation history (i.e. red book) with you when you attend.

We offer immediate and necessary treatment for those visitors to the area who have a medical condition that requires immediate attention.
